Norma Ann Phillips Hayes, 72, of Madisonville, Ky. passed away at 9:15 a.m. on Friday February 18, 2011 at her daughters home in Henderson.

She was born on October 14, 1938 in Ozark, Ill. to the late Mina Mae Phillips and James Hershel Phillips. She was formerly a nurse aide with Henderson Manor. She was of the Pentecostal faith.

In addition to her parents she was preceded in death by her ex-husband, Norman Clark Hayes; a son, Craig Rash; a brother, Joe Phillips and a sister, Shirley Vincent.

She is survived by two daughters, Deborah (Jeff) Schindele and Mary Ann Edgell all of Henderson; three sons, Timothy (Rhonda) Hayes of Salem, Ky., Anthony (Frieda) Hayes and James Hayes all of Henderson; a brother, Jerry Phillips of Madisonville; five grandchildren, four step-grandchildren; one great-grandchild; and several nieces and nephews.

Funeral services will be held at 1 p.m. on Monday February 21, 2011 in the chapel of Harris Funeral Home in Madisonville with Bro. Don Johnston officiating. Burial will be in Rosecreek Cemetery.

Visitation will be from 11 a.m. until the funeral hour Monday at Harris Funeral Home.
