Carolyn Cates Falk (Faulk), 66, of Madisonville, Ky. passed away at 12:58 p.m. Friday February 12, 2010 at Regional Medical Center in Madisonville.

She was born on October 9, 1943 in Nebo, Ky. to the late Sherman Cates and Margaret Dorris Cates. She was a homemaker and was a member of United Church of Jesus Christ.

In addition to her parents she was preceded in death by her husband, William "Freddie" Falk (Faulk) on March 25, 2006; a sister, Bobbie Joyce Cates and a brother, Don Cates.

She is survived by two sons and daughters-in-law, John and Sandra Faulk, William "Eric" and Donna Faulk all of Madisonville; one daughter and son-in-law, Tara and Larry Fryar of Madisonville; a sister, Joan Scott of Diamond, Ky.; five grandchildren, Mark(Kayla) Faulk, Beth Faulk, Erica Faulk, Amanda Daugherty, and Daniel Stevens; three step grandchildren, Brandon Austin, Christopher Austin and Michael Fryar; three great grandchildren, William Lane Faulk, Kaleigh Faulk and Addison Fryar; several nieces and nephews.

Funeral services will be held at 2 p.m. Sunday February 14, 2010 in the chapel of Harris Funeral Home in Madisonville with Rev. Alan Hopper officiating. Burial will be in Grapevine Cemetery in Madisonville.
